I'd also note that although Richard Castellano (Clemenza) is no long with us, Abe Vigoda (Tessio) is still around at the age of 89 -- old, grey, but as far as I know, not an Eagle. It's clear enough that "caporegime" is head of the regime (or organization). For the relationship between "head" and the thing that goes on the guitar, see http://en.wikipedia.org/wiki/Capo.
