Help with Conservation Badge

We are just about done with our conservation badge is there some special form I need to fill out and send in. I thought I heard that somewhere.

I simply reported to our CC which boys had completed their Conservation Patch and she had them ready at the next pack meeting.

If you mean the Cub Scout Conservation Good Turn Award, you can get a form for that from your local council office or see the link at the bottom of http://www.boyscouttrail.com/content/Award/Award-240.asp

That's quite an official looking application form. Can you enlighten me on why you would need it? Our council store just sells the patches to anyone who wants one, no application required.

My council shop requires the form. I had no idea till I went to get the patch. I am not sure why there would be some who require the form and some who don't. Maybe just another way of making the norm hard to follow as one shop worker said we didn't need it but was quickly corrected by another. Anyone know when the form was created? Is is new or old and being fazed out?
