What kind of team? School team or rec league/other team?
Some school conferences require that a player make every
practice or a certain number of practices in order to play
that week. Some rec league teams have the same requirement
as a "fairness" issue to prevent Superstar Johnny from
playing every down although he never comes to practice.

Not germaine. See above.

usually meas (1), sometimes means (3), (2) would mean he was
"cut" from the team.

Doesn't that depend primarily on the quality of the
individual's play rather than attendance?
The thing we may be forgetting is that Scouting is not
competitive; and, our "season" is 52 weeks long. "Everyone"
makes the team. Scholastic and select teams may be highly
competitve for both participation and playing time and, the
seasons may only include 8 or 10 games.
