Many (most?) of the scouts in our troop have such a book and we recommend
it to all new scouts. However, they have put it together themself. That
way then get the experience of figuring out what THEY need to keep track of
and how they can best do so.

I suspect that people have trouble using the book because they didn't write
it. So by all means, put together and example of such a notebook and show
it to your scouts, but please don't take away from them the chance to learn
how to organize themself.
