There are actually four Surgeons General, but you probably mean the SG of
the US who is part of the Public Health Service. My guess is that, if all
else fails, you should be able to address it as:

Surgeon General of the United States
5600 Fishers Lane
Rockville, MD 20850

but try the phone number 301-443-4000 first for a perhaps better address.

Maybe Bruce Cobern can get you a good address here, he lives in the Apple.
