My Troop always uses them. Along with the standard name, address, and
telephone number, we've recently begun to include a space on the form
for the parent's Social Security Number. We've discovered that the
boy's folks are often not at home when we call, and that the hospitals
are *real* reluctant to treat a Scout without that darned number in
their database.
