This brings up another interesting point. I have been told that about
the only good a medical form does is give the hospital any pertinent
information (immunizations, allergies, exsisting conditions etc.) that if
the child needs treatment they will still seek out the parents
permission (even if only by phone). So a xeroxed copy will still have
all the info on it. My daughter was taken to the emergency ward from
resident camp a few weeks ago with a severe cut on her leg. I was first
contacted by the camp nurse asking permission to take her to the
emergencey ward, then I was contacted by a nurse at the hospital, asking
permission to give her a tenus shot (she was on the borderline of needing
a booster). She didn't need stitches only those sterile strips all of
which the hospital nurse explained over the phone. Then they sent her
back to camp. When I called our doctors office (immediately after
receiving the first phone call) they told me the hospital would at the
minimum be calling me, and if she needed to be admitted, I would have to
be there (like I wouldn't be there in a hurry anyway even if my car was
parked an hour away!).
Is this something that varies from state to state or is it the policy
everywhere? In which case, I wouldn't see why that the signature xeroxed
or not would be that big of a deal on the medical forms, as long as all
the important information is legible.
