Proper flag disposal

Dave DesMarais, a recent Eagle and brand-new ASM in our troop came to the
monthly Scoutmaster's meeting last night with a request that I hope List members
can help address. He's been asked by a Pack leader to help destroy a worn out
US flag at an upcoming Pack Campout and Cross-over ceremony. The question is:
what is the "proper" and respectful way to incinerate a flag for disposal? Any
recommendations will be gratefully accepted and should be sent privately to:
