100% top trained leaders

In the back of the BSA publication "Training Plans, Procedures and
Policies " (I think that's the name) there are a couple of pages on
how a district achieved nearly 100% trained leaders. However, it is
rather vague on some of the critical details.
It appears that the district referenced was in the Detroit area. Is
anyone on the list familiar with that situation?
I'm specifically interested in: When was this accomplished? Were
all BSA programs included in the totals? How big was the district?
How long did the statistics remain that high? Was this just top unit
leaders (SM, CM, etc.) or all leaders? What was the overall trained
leader % at the time?
