Because the section deals with ADULT leadership, Henry, not "adult and youth
leadership".
Here's the "unbolded" (the bold part is the part I posted earlier,
which is policy; the unbolded part explains the policy:)
"If an accident occurs, one person stays with the injured, and two
go for help. Additional adult leadership requirements must
reflect an awareness of such factors as size and skill level of
the group, anticipated enviornmental conditions, and overall
degree of challenge."
Because you NEVER want a situation whereby you have "one on one",
the "one person stays with the injured" should read "two people
stay with the injured", and particularily if the person injured is
a female youth member.
So, in reality, you have two sets of adults (not necessarily
two couples, but two males and two females) of which two would go
for help, and two would remain, maintaining the BSA's "two deep
rule".

That by itself doesn't "require it", but if you add the last
sentence of the two-deep leadership policy, it becomes clearer:
"Coed overnight activities require male and female adult leaders."

Nope. I think that the Safe Guide for Scouting, as we've talked
about before, need to be explained a lot clearer and SPECIFICALLY
state what's expected of adult leadership for Cub, Scout and
Exploring events. I understand that Exploring is a much smaller
part of our program than Cubbing and Scouting, but the basic
safety book should explain it (and illustrate it as well).
