Belt Loops/Pins - A New Line

I originally posted the email about how dens and packs administer the
belt loop/pin program.
Now I would like to follow a slightly different line of discussion.
First when was the belt loop/pin program implemented?
Second, it seems to me that the program is poorly managed at the
national level. The books are poorly written with references of one loop in
another book (see physical fitness book at the back referring to archery).
Third, the belt loop can be worn on the belt, but the pin which many
times requires much more work (especially the achedemic ones) than many of
the traditional awards (arrow points) are relegated to the hat. Why can't
the pins be worn on the left pocket flap which is currently empty?
