Guide to Safe Scouting is usually the first place I look to for Health
and Safety policies (not guidelines, but "gottas"). It is easier to
read than the rules & regs and charter & by-laws, but I also refer to
those.

Medical Info.:

Personal Health & Medical Records, Class 3, No. 4412A is to be used
for Class 3 activites. Adults will use this form for Class 2 AND
Class 3 activites.

CLASS 2:

(Remember, these are in bold print in the pamphlet, which means they
are policies, and must be followed. Local councils can not change
them to less than required.)

CLASS 2:
....any even that EXCEEDS 72 CONSECUTIVE HOURS, where the level of
activity is similar to that normally expended at home or at school,
and where medical care is readily available. EXAMPLES: resident
camping, tour camping, and hiking in relatively populated areas.
Medical data required is AN ANNUAL HEALTH HISTORY SIGNED BY PARENT OR
GUARDIAN SUPPORTED BY A MEDICAL EVALUATION COMPLETED WITH THE LAST 36
MONTHS BY A PHYSICIAN LICENSED TO PRACTICE MEDICINE. The Personal
Health & Medical Record, Class 2, no. 4414, is designed PRIMARILY for
resident Cub Scout and Boy Scout summer camp but could be used for
asny Class 2 acitivty. ONLY youth members use this form. The Class
requiremet or from is not applicable to adults.

CLASS 3:
..... any event involving strenuous activity or conditions usch as
packpacking, high altitude, exttrreme weather conditions, cold water,
exposure, fatigue, athletic competition, adventure challenge, or
reomote conditions where readily avaailable medical care cannot be
assured. EXAMPLES: high-adventure activites, jamborees, Explorer
olympics, and extended backpacking trips in remote areas. Medical
information required includes CURRENT HEALTH HISTORY SUPPORTED BY A
MEDICAL EVALUATION WITHIN THE PAST 12 MONTHS PERFORMED BY A PHYSICIAN
LICENSED TO PRACTICE MEDICINE. Personal Health & Medical Records,
Class 3, no .4412A, is to be used for Class 3 activites. ADUlTS WILL
USE THIS FORM FOR CLASS 2 AND CLASS 3 ACTIVITIES.
