In regards to Cub Scouting, sample text for a den activity permission
slip is included in the Cub Scout Leader Handbook. Use of any permission
slip also requires that you follow all other BSA rules and regulations.
This would also require you to file other permits as needed (camping,
tour, etc). Remember that for BSA also, you need to be careful to
differentiate FAMILY activities from official PACK or TROOP activities.
Certain things are NOT allowed at official activities but are allowed
on FAMILY actitivites (i.e. swimming without a person certified in
water safety or lifesaving). The difference may seem small, but it could mean
the DIFFERENCE on whether you are covered with BSA, Unit, or individual
insurance. You should always have with you copies of the medical
information for each boy in case of a real emergency if you are on
OFFICIAL functions.

The simple rules here is 1. Establish the scope of the ativity (BSA or
(FAMILY). 2. If official, be sure that all BSA rules and regulations
for that activity are followed including proper leader and child ratios,
youth protection guidelines, and proper permits and insurance. 3. If
family, be sure that each youth is accompanied by an adult (I believe
simply over 18 - if not, correct me.. ;) who is a part of the youth's
family or approved by them.

If you fail to do at least 1 and 2 for any OFFICIAL activity, you could
end up being personally liable if anything happens. Be prepared.
