Hey Ron!
I've got two or three versions of a "suitable" permission slip which
should answer what you need....I'm downloading all of last month's mail today
so, I'll post the versions later on today.
I'll also check the Scouting resources here and see if I can find one in
which I can "point" to as an example of a "good permission slip".
Note please, that there's a difference between a "permission slip" and the
thing that I am sure that your parent is wanting, called a "emergency
treatment form" (or release). There are some minor differences between the
two...also note that as long as he's filled out the BACKSIDE of the Scout's
application, that such "treatment release" is NOT needed....please note
what the backside of the application says:
"PARENT AUTHORIZATION"
This health history is correct so far as I know, and the person herein
described as permission to engage in all prescribed activities, except as
noted by me. In the event I cannot be reached in an emergency, I hereby
give permission to the physician, selected by the adult leader in charge, to
hospitalize, secure proper anesthesia, or to order injection for my son.
(signature of parent or guardian, date, home and business telephone numbers).
LIABILITY release is something else...and as I stated, I'll find what I can
from my file cabinet here for you.
