For the last several years we have used the following form in Troop 25. It's a
variation
of what I found in one of the files on the list-server. The prior Scoutmaster u
sed no
permission slip at all and the parents had only very sketchy information about t
he trips.
Preparing for a trip and buying food was haphazard at best.
