Over the years, we have used a "one year" form that has all the
required treatment permission stuff plus the listing of our Scouting insurance
policy. We have carried them in a binder with the person's medical form
and a copy of our current BSA insurance policy...
BTW, if you do not file a tour permit with your council, "Kiss any claims
on your BSA insurance policy goodbye".
