Premission Slip Text

For the last few years, I have been using the enclosed text on my
permission slips for most Troop events:
"_____________________________ (Scouts Name) has my (our) permission to
attend the <SHORT-EVENT> from <LEAVE> through <RETURN> with Troop 91. In
consideration of the benefits to be realized by him from this experience,
we waive all claims of responsibility for mishaps to
_____________________________ (Scouts Name) against all Unit Leaders, the
Sponsoring Institution, the Northwest Suburban Council and the Boy Scouts
of America.
"Signed:_________________________________________ On: ________________________
"Please bring this form to any Scout Meeting on or before <LAST_MEETING>."
Most of the time, these slips have been willingly signed by the parents,
and have been sufficient (we have not had any injuries etc.)
However, before the last campout, the father of one of my newest scouts
expressed some concerns that this form did not provide for a permission to
treat, and waived all liabality claims. He was willing, fortunatly, to
write up a seperate slip for his son.
This got me thinking about how premission slips should be worded. Since I
am not a lawyer, and do not want to spend lots of money to have a lawyer
word the premission slip, I am aware that these slips are of little leagle
value anyway (i.e. I can still get sued even with the parent's signature).
Unfortuatly (and probably deliberatly) national does not provide a
suggusted text for this (at least not in the Scoutmaster's Handbook, Wood's
Wisdom and I think the Explorer Leader's Guide). So, I figgured I'd ask
(yet again) the wisdom of the list for suggustions for the text of a
premission slip that will satisfy most parents, protect (as much as
possible) the Troop leaders et. al., and provide for getting an injured
scout treated.
