We have a permission form that all members of the Post and adult
advisors/participants are required to sign. It is kept in a word
processor and is changed to describe the specific events, risks, dates,
locations, etc. A Scouter/Attorney assisted us in the design of the form.
THe form also has the standard medical consent, informed consent and
liability waiver, health insurance info, and a signature block for the
participant, the parents if under 18, and the advisor approval.
It is important to have a consent form specific to each activity which
describes the nature and risks or potential hazards of the activity, and
all participants, youth and adult, should sign it.
This way, if an emergency medical treament consent is needed and parents
cannot be contacted, for those under 18, the physicians have a go ahead,
although in practice they will make every effort to contact parents
directly by phone prior to initiating any treatment other than emergent
lifesaving measures, which they actually can do without consent anyway.
It is important to verify signatures and be sure that parents have
actually signed the document for those under 18, in case something should
happen and the documentation would be questioned if litigation were to
arise, or if medical consent were needed where parents could not be
contacted.
