Info? Qulaity District patch

Hello list- A few months ago, someone mentioned a source of Quality
District patches for those of us directly registered a the District level.
I printed the address and put it "away". Now, I can't find "away" again. It
isn't lost just somewhere. Can anyone re-supply the information to my
private address?
