A brief answer to Ken Speigel's multiple question:

Yes, a Scoutmaster may refuse to issue a merit badge application and thus
refuse to provide the name of a counselor. BUT, there is only one acceptable
reason for this:
The Scoutmaster determines if the Scout is prepared to earn the merit badge.

Whether or not it is an Eagle required merit badge is irrelevant. The
Scoutmaster's evaluation on the Scout's readiness for Eagle is not relevant
to the earning of merit badges either and is better addressed in other ways.

Impeding a Scout's self-determined merit badge program for any reason other
than a Scout's lack of preparedness is a simple way of saying "Go to another
troop, kid, your botherin' me!" And that is what he may do (and I would hope
he does).
