things parents don't tell you

As part of my Wood Badge ticket, I came up with a way to
better "know your resources" and made a questionaire about
each boy. As the parents of new scouts came into the troop
I sat down with them and we went over this questionaire.
Nobody objected to it and it gave me better insight into the
boys and gave me some other information the medical history
form did not. Here is a jammed together version of the
questions. They fit easily in two colums on one page,
double spaced:

Name:
Date and place of birth:
Family Doctor:
Siblings & ages:
Any special details about siblings?
Does he easily talk to you about "stuff?"
Does he confide in you?
Scholastic strengths (classes he like):
Scholastic weaknesses (classes he has trouble with):
Interested in any sports?
Special talents or skills he's proud of:
What would he like to start doing?
What would he like tostop doing?
How is his free time spent?
Hobbies or liesure activities:
Any phobic fears? (bees, snakes, heights)
Any special illnesses or chronic diseases?
Does he make friends easily?
Hashe ever been bullied by another youth?
Is he/does he: assertive lazy eating problems
withdrawn temper tantrums
overeat have a fear of failure
Current medications:
Does he eat three balanced meals a day?
Does he exercise regularly?
Does he bruise or bleed easily?
Is he a doer (keeps busy, like to get things done)?
Does he daydream?
In a group of friends, does he lead or follow?
Does he like scouts?
Does he have a favorite scout for a friend?
What can I do to make his time in Scouts more enjoyable?
Is he afraid of any other boy or adult leader in the troop?
Aspirin, Advil, Tylenol or nothing?

I had a psychologist help me with the questions. It is a
great suppliment to the medical form.
