after event reports

Hello all,

I was wondering if any units out there have a standard form used by
adults and youth that is filled in to recap an event.

I was thinking about building a notebook of these for future reference
and also for program planning, and also handing them out at roundtable
to other units.

Items that might be included would be:

location/including directions from a central point in district
type of event/activity-(base camp/backpacking
moderate/strenuous,canoeing,whitewater, etc)
restrictions
contacts/costs
what was good
what wasn't good but could have been done better


If anyone already uses such a form like this, I would be interested to
see it or if you have ideas for what to put on a form like this please
forward me your ideas and I will try to make such a form.
