There is a separate BSA health form for adults. I have used it for summer
camp
and Philmont.

This must be a local variation, maybe depending on state laws. At our
summer camp
in Michigan, the scouts can use the form with one physical exam for three
years. But the
parents have to sign each year to certify that the health history hasn't
changed, etc.
