Belt Loops and Pins A New Line

I posted an email earlier about how packs and dens administered the
belt loop/pin program. I would now like to ask for some background on the
program if you please.
First of all, when was the program implemented?
Second, it seems to me that it is ill managed at the national level.
The books seem to be written poorly with reference to an unrelated belt
loop, see physical fitness book at the back.
Third, the belt loop can be worn on the belt, but the pins are
relegated to the hat. Many of the pins (especially the achedemic ones)
require much more work than most of the regular awards (such as arrow
points) and yet cannot be worn on the regular uniform shirt. I think they
should be worn on the left pocket flap which is currently empty.
Can you much more experienced folks help out this poor first year den
leader?
