tipping

Hi All
I will be travelling again in June and this time it involves the USA. I am
wondering about the ettiquette of tipping in that county as for here in
Australia it is almost unheard of. What warrants a tip, what percentage of
the bill should it be.
Australians have a notorious reputation for being lousy tippers because this
is not part of our culture, it is something that someone else does.
Thanks in advance
Mark
